Houston Methodist Covid-19 Vaccine Interest: 50+ ONLY

Houston Methodist invites you to receive the COVID-19 vaccine if you are 50 and older. If you fill out this form, they will soon contact you via text message to help you schedule a vaccination appointment. In the text, you will receive a contact number if you need assistance with scheduling. If you do not have a mobile number, you will be contacted at your home number to schedule. If needed, Houston Methodist can also assist with transportation. Thank you for joining our fight to stop the spread of this virus. You must be 50 years or older to be eligible for scheduling.

Thank you for expressing interest in receiving the COVID-19 vaccine from Houston Methodist. We recently found out that Houston Methodist is prioritizing scheduling individuals who are age 50 – 64 with a pre-existing chronic medical condition. This means that if you are 50 – 64 without a pre-existing condition, you will not be able to move forward in the scheduling process when you try to use the link provided to you via text message or email. Houston Methodist sincerely apologizes for any inconvenience. If you are age 50 – 64 without a pre-existing condition and, therefore, unable to schedule, Houston Methodist will maintain your information on their waitlist & when they are able to expand beyond the underlying conditions criteria, you will be re-invited to schedule a vaccination appointment with them at your preferred Houston Methodist facility.

Please note, Houston Methodist will allow you to schedule your appointment if you are 50 – 64 with one or more underlying chronic medical conditions as this makes you eligible through the Phase 1B criteria. Please review the Texas Phase 1B eligible conditions below:

  • Cancer
  • Chronic Kidney disease
  • COPD (Chronic obstructive pulmonary disease)
  • Down syndrome
  • Heart conditions, such as heart failure, coronary artery disease or cardiomyopathies,
  • Solid organ transplantation
  • Obesity and severe obesity (body mass index of 30 kg/m2 or higher)
  • Pregnancy
  • Sickle cell disease
  • Type 2 diabetes mellitus
